Submission timestamp — the exact date and time the form was submitted.
UTM source — the traffic source (e.g. google, facebook, email).
UTM medium — the marketing channel (e.g. cpc, organic, referral).
UTM campaign — the campaign name.
UTM content — used to differentiate between ads or links in the same campaign.
UTM term — the paid keyword, if applicable.
UTM referrer — the full referring URL.
Address — if the form used a postcode lookup, the parsed street, town, and county are stored alongside the raw answer.
Map URL — a Google Maps link to the lead's location, captured when coordinates are available from an address lookup.
Updated by — the name of the admin who last changed the submission's status, if anyone has.
Leads table — Source, Campaign, and Medium appear as dedicated columns. Toggle them on via Customize Columns if they are hidden.
Submission drawer — the Campaign Information section shows all six UTM fields when any of them are present. The address and map sections appear when location data was captured.
CSV export (table) — every field is included in the download: ID, Form ID, Status, Map URL, one column per question, UTM Source, UTM Campaign, UTM Medium, UTM Content, UTM Term, UTM Referrer, Submitted At.
CSV export (per lead, from the drawer) — the same columns as the table export, plus an Address column.
